Pijn is de meeste voorkomende klacht waarmee de patiënt naar de dokter of tandarts gaat. In deze presentatie gaat prof. Albert Dahan in op onderstaande onderwerpen en bespreekt hij één of meerdere casussen.

- De pathofysiologie van pijn met focus op het endogene pijnsysteem verantwoordelijk voor placebo analgesie, stress-geïnduceerde analgesie en het verlies van endogene pijnstilling in chronische pijn en dementie. Ook zal stil gestaan worden bij het concept van hereditaire analgesie.
- De diagnostische mogelijkheden met aandacht voor conditioned pain modulation en offset analgesia. Deze testen geven een indicatie van het endogene pijnsysteem.
- Behandelopties, zowel medicamenteus (opioïden, anti-epileptica, antidepressiva, NSAIDs) als niet-medicamenteus (fysiotherapie, cognitieve gedragstherapie), binnen het kader van de opiaatepidemie.
- Effecten op lange termijn (chronische of persisterende pijn).

Traumatic events have immediate impact on the affected teeth, but may also lead to delayed and long term consequences that again can put tooth survival in jeopardy, or at least impair its full recovery. 

Endodontic treatment is often recommended, but its indications are sometimes unclear, as are the timings for performing the treatment. Diagnostic difficulties arise, knowing that some teeth do not respond to stimuli for a while after trauma, and others may have an exaggerated response for some time immediately following the traumatic event. However, a diagnosis as accurate as possible is needed to help choosing the best available treatment.

During this presentation, an evidence based aproach will be shared with the audience. Selected classic and recent literature, as well as the recently updated dental trauma guidelines will be reviewed, supported by clinical examples. Simple and complex cases will be presented and discussed, from both the dentist and the patients point of view, contributing to understand the basis of diagnosis and treatment plan.

Kroonverkleuring is een esthetisch significant gevolg van dentaal trauma. Het is de externe vertoning van pathologie in het pulpa-dentinecomplex. Hoewel het een teken is van interne schade, vertaalt het zich in wezen niet in onomkeerbare schade en kan het soms van voorbijgaande aard zijn.

Tijdens deze lezing worden de mogelijke behandelingen voor de verscheidene types tandverkleuringen na trauma besproken alsook de risico’s die gepaard gaan met de behandelingsopties.

Een dentaal trauma kan ontstaan door een externe of interne inwerking van krachten. Een extern trauma is vaak een eenmalige inwerking van een grote kracht. Een intern trauma is een meestal herhaaldelijke inwerking van kleine krachten veroorzaakt door ongunstige contacten tussen gebitselementen. Na een extern trauma en het (dreigend) verlies van een of meerdere gebitselementen staan we vaak voor het dilemma of we de ontstane ruimte openlaten en gebitselementen reconstructief vervangen of dat we de ruimte sluiten. De uitdaging bij een intern trauma is van een heel andere aard. Vaak wordt een intern trauma te laat of in het geheel niet ontdekt en worden alleen de symptomen behandeld zonder de oorzaak aan te pakken.

Deze voordracht zal duidelijke richtlijnen geven voor de diagnostiek en de behandeling van gebitssituaties na het ontstaan van intern en/of extern trauma. Voor- en nadelen van de verschillende behandelopties worden aan de hand van verschillende klinische voorbeelden getoond.

Het plaatsen van hechtingen in de mond is niet altijd vanzelfsprekend en kan vragen oproepen.
- Plaats ik een hechting na een extractie van een melktand?
- Moet ik anders hechten indien de patiënt bloedverdunners neemt?
- Kies ik hier voor een spontaan resorbeerbare draad of net niet?
- Hoe plaats ik weeral een doorlopende hechting?

Een goede basiskennis over wanneer wel of niet te hechten, type draad, soort hechting en verdere opvolging is onmisbaar in de tandheelkunde.

In deze praktisch georiënteerde sessie focussen we ons op hoe vlot te hechten en leren we de basis aan om hechtingen te plaatsen. Aan het einde van deze workshop moet u voldoende kennis hebben om zelf uw type draad en materiaal aan te kopen, in te schatten wanneer u wel of niet moet hechten, vlot te hechten en de juiste techniek toe te passen in elke basis situatie.

In deze voordracht wordt de farmacologie van analgetica die gebruikt worden in de postoperatieve setting, waaronder het gebruik van pijnstillers en ontstekingsremmers na een tandingreep, besproken.  In de inleiding wordt het verschil geduid tussen acute, postoperatieve pijn t.o.v. chronische (vb. kankergerelateerde) pijn en neuropathische pijn. Aan de hand van de pijnladder van de WHO wordt de farmacologie van paracetamol, NSAIDs en (zwakke en sterke) opioiden besproken, waarbij er gefocust wordt op het correct en veilig gebruik in de dagdagelijkse praktijk.  Er wordt vooral stilgestaan bij een aantal praktische aspecten zoals aanbevolen en maximale dosering, contra-indicaties en interacties.

In een tweede deel wordt ook een globaal overzicht gebracht van de geneesmiddelen die gebruikt worden bij chronische en neuropathische pijn, gezien deze geneesmiddelen frequent als chronische thuismedicatie worden gebruikt, ook door patiënten die een tandingreep ondergaan.

Tijdens de lezing van krijgt u inzicht in de wereld van fotodontie. In 90 minuten bent u bij en op de hoogte van alle mogelijkheden om de volgende werkdag snel en goede dentale foto's te maken. Daan van Oort  bespreekt:

- Welke camera's geschikt zijn voor dentale fotografie. Niet alleen de spiegelreflex camera komt aan de orde maar ook het fotograferen met uw mobiele telefoon in combinatie met extra ledlichten, de shofu eyespecial C-IV en de Medical-D.
- Hoe u een apparaat goed instelt om snel en gemakkelijk mooie beelden te maken.
- Wat er te koop is aan hulpmiddelen en met welke kwaliteit foto's u een behandelplan kan maken.
- Waar op te letten bij het fotograferen van dentale portretten aan de hand van voorbeeldfoto's.
- Het goed gebruik van spiegels en retractors opdat mondfoto's voortaan met speels gemak kunnen worden gemaakt.
- Waarom beelden met wifi overzetten niet vanzelfsprekend is en of er een oplossing is.

The purpose of the lecture is to make a critical appraisal of modern materials and procedures to be used in case of a dental trauma, including dentin bonding agents, resin composites, color and aesthetics principles, and the step by step procedures for direct anterior restorations and for fragment reattachment. Many different aspects of multidisciplinary treatment planning will also be discussed.

A dental trauma is always an event that creates a lot of stress or patients and for dentists. Understanding the cause and the effect of the trauma contributes to a correct diagnosis and helps the dentist making a correct treatment plan. Adhesive technologies have contributed to change the way dentists conceive aesthetic restorations. The silent revolution of dentin bonding agents has allowed practitioners to minimize their preparations: it is now possible to obtain an “invisible” restoration in terms of color, shape and surface texture. Due to the improvements of materials and techniques, dentists can restore aesthetic appeal and function with a low biological and financial cost.

Patients are aware of the new technologies, and require treatments that are highly aesthetic, minimally invasive, repairable and not too expensive. Resin composite restorations can fulfill all these requirements. However, the correct protocols have to be implemented to achieve clinical success and to obtain invisible restorations.

Whenever the tooth fragment is available, it should be reattached following the moderm techniques, which ensure a long term clinical success. Multidisciplinary treatment planning is most often the key to success: many patients need a combination of different treatments to solve their problems. It is extremely important to combine the skills of several specialists to establish the most constructive treatment plan to ensure the best possible result for the patient.

Wanneer de tand gerelateerde toestand van een persoon omwille van één of andere gebeurtenis in een toestand werd gebracht die minder is dan de toestand waarin deze persoon zich zou bevinden zonder dat die gebeurtenis zich voordeed, spreekt men over tand gerelateerde schade.

Het takenpakket van de tandarts beperkt zich niet tot het behandelen en herstellen van deze schade. Immers, wanneer derden betrokken waren bij de gebeurtenis die de schade tot gevolg had, kunnen deze hiervoor aansprakelijk worden gesteld. Juridisch dient het slachtoffer hierbij het bewijs van zijn schade te leveren. Dit kan niet zonder de hulp van zijn tandarts. Die kan het bewijs leveren door middel van een eenzijdige tandheelkundige expertise. Soms wordt de tandarts zelf aansprakelijk gesteld voor ontstane tand gerelateerde schade en dienen specifieke maatregelen te worden genomen.

In deze presentatie zullen de betreffende juridische basisbegrippen kort worden uitgelegd. Vervolgens zal uitvoerig worden ingegaan op gerelateerde taken van de tandarts. In het bijzonder op welke manier beschikbaar bewijs van tand gerelateerde schade dient te worden geregistreerd via een eenzijdige tandheelkundige expertise en door middel van het patiëntendossier. Verder zal worden besproken hoe de patiënt dient te worden geïnformeerd over geplande behandelingen. Er zal worden ingegaan op maatregelen die de tandarts kan nemen om klachten met betrekking tot uitgevoerde behandelingen te vermijden en welke stappen kunnen worden ondernomen wanneer de tandarts aansprakelijk wordt gesteld voor mogelijks ontstane tand gerelateerde schade. In een interactief gedeelte zullen specifieke schade-casussen worden voorgesteld en besproken.

Autotransplantation of teeth before the root is fully formed is an established treatment option for replacing missing teeth in growing individuals. Favourable orthodontic indications, good surgical access and optimal tooth morphology are mandatory for a successful outcome of the treatment. Premolars with developing roots due to their morphology and position in the dental arch are optimal donors with reported success rate of over 90 per cent. Autotransplantation of developing, unerupted premolars to maxillary anterior region and to replace missing mandibular premolars will be presented. The description of the surgical technique and factors, which may influence the treatment outcome will be given and discussed.

The lecture will summarize the knowledge about the protocol for autotransplantation of developing teeth, the treatment planning, the healing of autotransplanted teeth and the outcomes of the treatment for different groups of teeth.

On top of that, a natural potential of transplanted teeth for bone preservation and regeneration at sites of transplantation will be demonstrated, using clinical examples, that those teeth may in many cases serve as a natural providers of bone growth and may help in regeneration of the alveolus in patients with bone defects. This advantage of the treatment seems to be the most strategic factor in favour of performing tooth autotransplantation of teeth in children in order to avoid the need for implant placement in the future.

Patients with varying severity of tooth loss can be rehabilitated by different types of removable prostheses, which are retained by the remaining natural teeth or directly by the oral mucosa.  In the era of dental implants, everyone seems to be chasing the ultimate tooth replacement, i.e. fixed tooth replacement using dental implants.  However, there are often anatomical or patient-related factors in which fixed implant prostheses are contraindicated.

This presentation will aim to discuss the role of removable prostheses in replacement of missing teeth in the era of dental implants and the demands of modern patients.

The replacement of a missing maxillary incisor with an implant restoration is a routine dental procedure. As with every successful 3D regeneration of the implant’s ‘bony housing’ in the esthetic zone, it is mandatory to augment the soft tissue to ensure the buildup of a ‘soft tissue housing’ with high functional and esthetic qualities. However, in most cases the regenerated bone dimensions may be limited by anatomic characteristics and other variables. To compensate for the partially missing bone it is necessary to augment the soft tissue component more than is usually needed.

In this presentation,soft tissue grafting procedures utilized in such cases will be evaluated for their ability to maintain the desired peri-implant soft tissue function and architecture long term.

Root fractures are infrequent injuries, comprising 0.5 to 7% of the injuries affecting the permanent dentition, and 2-4% in the primary dentition. This type of fractures involve cementum, dentin and pulp. The mechanism of a root fracture usually is a frontal impact which creates compression zones labially and lingually. The teeth most affected are permanent maxillary central incisors. The fracture line (or lines) may be well within the bony support, in which case it is considered a deep root fracture, or the fracture line is at or close to the crestal bone of the alveolus and is called a shallow root fracture. Diagnosis entirely depends on radiographic examination. The principles of treatment in permanent teeth are reduction of displaced coronal fragments and splinting. Several clinical reports have demonstrated success after root fracture treatment; however follow-up examinations are advised.
